VRC Intake Step-by-Step Guide
Documents You’ll Need
If you’re a Veteran:
- DD-214 Member 4 or Military ID
- Certificate of Eligibility (COE) or NOBE (DD-2384) for Reservists, or 28-1905 for Voc Rehab (Ch 31 (optional)
If you’re a Dependent, using benefits:
- VA Certificate of Eligibility (COE) or CalVet Award Letter
- Qualifying Veteran’s DD-214 Member 4 (optional)
If you’re a Dependent, not using benefits:
- Military ID
OR
- Qualifying Veteran’s DD-214 Member 4 AND
- Marriage Certificate (Spouse)
- Birth Certificate (Child)
- Birth Certificate and your parent’s Marriage Certificate (Stepchild)
Once you click the link to the form, you’ll use your Cypress College Single Sign-On to get started! You should see the welcome screen below:

Once you click “Complete This Form,” you’ll be redirected to the first page. Your first and last name and your Cypress ID will auto-populate, you’ll just need to add responses to the blank boxes and select Veteran or Dependent:

Then, click “Submit Form”!
If you selected Veteran:
You’ll land on the Veteran Intake Form and start by telling us if you plan to use benefits:

If yes, you’ll be prompted to select which one you plan to use. If you’re not sure which one yet or you’d like more information on your options, select “Unsure.”

If no, you’ll skip down to the next section where you’ll upload your Intake Documents. Please note that only your DD-214 or Military ID is required.

Next, you’ll let us know if you have attended any other colleges! This will be important for us to know, especially if you plan on using benefits.
If yes, you’ll be prompted to list the names of the institutions and upload your unofficial transcripts if you have them. If no, you’ll skip down to the next section about Joint Services Transcripts (JST) Retrieval.
Your selection will determine our ability to retrieve your JSTs and review them for Credit for Prior Learning (CPL) eligibility.

Then, you’ll click the Signature line to sign and certify that the form is correct and click “Submit Form” at the bottom to submit!
If you selected Dependent:
You’ll be prompted to specify your relationship to the veteran before moving on to your corresponding form.

Based on your selection, you’ll land on the Spouse, Child, or Stepchild Intake Form and start by telling us if you’ll be using benefits.

If yes, you’ll need to provide the Qualifying Veteran’s First and Last Name and upload the VA Certificate Eligibility (COE) or CalVet Award Letter.
Please note that adding the Qualifying Veteran’s DD-214 is optional.

If no, you’ll have to indicate whether or not the Service Member is currently active. This will help us determine what alternative Intake Documents we’ll need to collect from you!
If the Service Member is currently active, and you have your own Military ID, please upload a copy of that to verify your dependent status.

If the Service Member is not currently active, please upload the requested documents based on your relationship to the veteran:

Next, you’ll let us know if you have attended any other colleges! This will be important for us to know, especially if you plan on using benefits.

After Submission
You’ll receive a submission confirmation to the email you added to the form immediately after submitting:

A VRC Support Staff Member will contact you to schedule your Intake Appointment.
Once you have confirmed your appointment date and time, you’ll receive a follow-up email with your appointment details and check-in information:
